The Community HealthCorps Hotline is designed to allow callers to share great stories, and report important other information to NACHC anonymously and confidentially. Calls are answered by voicemail, and checked by the Monitoring & Compliance Unit each business day. Callers who choose to leave contact information will receive a response within one business day. The purpose of Community HealthCorps' online helpdesk is to provide value-added technical assistance for local Community HealthCorps program coordinators. Benefits Summary: The Benefits Summary provides an overview of the healthcare insurance plan.

 

Benefits Contact List: The Benefits Contact List includes the contact information of benefit providers.

 

2010-2011 Injury & Sickness Insurance Plan: The 2010-2011 Injury & Sickness Plan provides a general summary of the insurance coverage.

 

How to Use Your Coverage MedRx: The guide instructs benefit enrollees on the process of using the coverage, and other resources that can be referenced.

 

Child Care Benefit for Community HealthCorps Members

Since 1994, AmeriCorps CARE has been the sole distributor of child care subsidies for members of the AmeriCorps national and community service programs throughout the United States and its contiguous territories. The Community HealthCorps Member Exit Survey

The web-based the National Community HealthCorps Member Exit Survey is short and very user friendly and available online through Survey Monkey. Completion of the national survey is a very important aspect of the exit process, whether exiting members:
• Earning an education award or not; or 
• Returning for another term of service.

Responses will be confidential unless exiting members indicate otherwise for specific survey items. Member deadlines for completing the online survey is set by program coordinators. Click here to access detailed instructions.

Community HealthCorps Benefits Summary: This summary is designed to give potential applicants an in-depth overview of the numerous benefits of serving with Community HealthCorps.  This document can also be used as a reference for current members who may have questions or want to learn more about the program benefits.

 

Community HealthCorps Templates

Community HealthCorps PowerPoint Template: This template should be used with NACHC Community HealthCorps-approved communications.  Further information regarding the template can be found in the Community HealthCorps Media Kit.

 

Graduation Certificates for Community HealthCorps Members: Follow these instructions to paste your own logos on it:

  1. Choose Tools > Advanced Editing > TouchUp Object Tool .
  2. Right-click the page and choose Place Image.
  3. Choose one of the following file formats: BMP, GIF, JPEG, PCX, PNG, EPS/PS, or TIFF.
  4. Select an image file, and click Open.

A copy of the image file appears in the center of the page, at the same resolution as the original file.
