What do Community HealthCorps members do?
Community HealthCorps members perform a variety of activities associated with health services and programs for patients and communities, often by functioning as community health workers (CHWs).
 
Examples of activities include:
·         Navigating the healthcare system
·         Helping people in need establish a “medical home” & improve utilization of health services
·         Individual & group health promotion/disease prevention education
·         Follow-up, case management, & support on treatment plans
·         Enrollment in public health insurance
·         Assessing other patient needs (financial assistance, housing, legal, etc.)

What are the goals of Community HealthCorps?
·         Increase access to and utilization of primary & preventive healthcare
·         Improve the capacity of health centers to provide quality & preventable healthcare services
·         Increase the quality & availability of preventive and primary care services & programs
·         Foster collaborations & partnerships that ensure the continuity and sustainability of programs & services
·         Encourage Community HealthCorps members to pursue further education & careers in community health through mentorship & experiential learning
·         Create a culture of civic engagement & volunteerism to strengthen preventive & primary healthcare
 
 QUALIFICATIONS
  • United States citizenship, national or lawful permanent   resident
  • 18 years of age, or at least 17 years old with a guardians signature
  • High school diploma or GED or commitment to obtain during your service year
  • Strong commitment to service
  • Respect & interest in working in medically underserved communities
  • Ability to work with diverse populations
  • Individual program sites may have additional qualifications

TERM OF SERVICE
 
Full-time service requires a minimum of 1700 service
hours over a 10 to 12 month period.
